Estrogen gets filed under "the period hormone," but it works far beyond your reproductive system. It helps look after your bones, heart, brain, skin and metabolism. That reach is why its rise and fall shapes how you feel at every stage, and why menopause touches so much at once. What estrogen does

Estrogen is a chemical messenger that switches many body processes on and off. Beyond running your menstrual cycle, it supports:

  • Bone density, helping guard against osteoporosis
  • Your heart and blood vessels, including cholesterol
  • Brain and mood, through its effect on brain chemistry
  • Vaginal and urinary tissue, keeping it supple and comfortable
  • Skin, through collagen and elasticity
  • Metabolism, influencing where fat is stored and how sensitive you are to insulin

That reach is why the drop in estrogen at menopause is felt across so many systems at once, from hot flashes to a shift in weight toward the middle and reduced insulin sensitivity.

How levels change through life

Estrogen is never static. It follows your life stage:

There are three types worth knowing: estradiol, the strongest and dominant through your reproductive years; estrone, the main type after menopause; and estriol, highest in pregnancy.

Estrogen across the cycle

Within each month, estrogen climbs through the first half of the cycle as an egg matures, peaks just before ovulation, then dips and rises again in the second half. It works in partnership with progesterone, which takes over after ovulation. When the two stay in balance, cycles are regular; when ovulation does not happen, that balance is lost, which is one reason periods turn irregular in perimenopause and PCOS.

What high or low estrogen can mean

Low estrogen, as in menopause, drives hot flashes, night sweats, vaginal dryness, mood changes, lower libido and bone loss. But menopause is not the only cause of low estrogen. Very low body weight, under-eating or heavy training can suppress it in younger women too, stopping periods and thinning bone, so it is not only a midlife concern. Levels that stay unusually high or low outside your normal life-stage pattern can point to a condition worth checking. Because estrogen interacts with so much, a symptom is rarely down to estrogen alone, and doctors read a result alongside your history and other hormones rather than as a single number.

Is it worth testing your estrogen?

Usually not, if the aim is to diagnose perimenopause or menopause. Estrogen swings so much day to day that a single reading tells you little. If you are over 45 with typical symptoms, doctors generally go on your symptoms and age rather than a blood test. Under 45, or when the picture is unclear, testing, often FSH rather than estrogen itself, can help. So if you are chasing a number, your symptoms are often the more useful guide.

What helps when estrogen is low

When low estrogen causes disruptive menopause symptoms, HRT replaces it and is the most effective option for many women. Current reviews favor starting within 10 years of menopause and using transdermal estradiol at low to moderate doses, which avoids the clot risk seen with oral estrogen (Int J Mol Sci, 2025), with vaginal estrogen a targeted choice for dryness. Whether or not you use it, strength training with enough protein, calcium and vitamin D protects the bone and muscle that estrogen used to support. Soy and other plant estrogens are sometimes tried for flushes, but the evidence is modest.

Book an appointment if you have symptoms of low estrogen that affect your daily life, irregular cycles that worry you, periods that have stopped when menopause is not the likely reason, or questions about your hormone health. Estrogen touches many systems, so it reads best in context, not from one lab value.
