Iron panel explained: ferritin, TIBC and transferrin saturation
What a full iron panel measures, what each marker tells you, why ferritin alone can mislead, and when to ask for the whole panel instead of one test.

An iron panel is a group of blood tests run in one draw: serum iron, ferritin, total iron-binding capacity (TIBC) and transferrin saturation. Taken alone, every one of them is easy to misread. Taken together, they usually show whether your body is genuinely short of iron, whether inflammation is distorting the picture, or whether something other than iron explains how you feel.
For most women investigating tiredness or hair shedding, ferritin on its own answers the question. The full panel earns its place in a narrower situation: when ferritin looks acceptable but your symptoms say otherwise, or when you have a condition that lifts ferritin no matter what your stores are doing.
What each marker on the panel measures
MedlinePlus describes the components plainly. Serum iron measures the iron in your blood. Transferrin is the protein that moves iron around your body. TIBC measures how well iron attaches to transferrin and other proteins. Ferritin measures how much iron you have in storage.
Transferrin saturation is not measured directly. It is calculated: the percentage of your transferrin that is actually carrying iron. Think of transferrin as delivery vans and saturation as how full they are.

Two practical points. Serum iron is the least useful marker on its own, which is why MedlinePlus notes your provider may ask you to fast for 12 hours and why the draw is usually done in the morning. And sTfR is not on most standard panels. The Clinical Medicine review is blunt about why: unlike ferritin, it is unaffected by inflammation, but the test takes too long and is not widely available.
Why ferritin alone can mislead you
Ferritin does two jobs, and that is the whole problem. It stores iron, and it also rises as part of your body's inflammatory response. It is an acute-phase reactant, meaning a chest infection, a flare of an autoimmune condition, obesity, liver disease or recent surgery can push it up while your iron stores stay empty.
The result is a normal-looking number hiding a real deficiency.
The published thresholds reflect this. In a woman without inflammation, the Blood Transfusion review reports that serum ferritin below 30 ng/mL is the most sensitive and specific cutoff for absolute iron deficiency. When inflammation is present, that line moves: ferritin between 30 and 100 ng/mL still suggests iron deficiency, and only above 100 ng/mL does the evidence argue against it. The Clinical Medicine review agrees, raising the threshold to 100 µg/L in chronic inflammatory conditions, and advises using transferrin saturation to make the call when ferritin sits in the 100 to 300 µg/L gray zone.
Ferritin is reported as ng/mL in the US and µg/L elsewhere. The numbers are the same.
This is not a reason to distrust your ferritin result. It is a reason to ask one question when it comes back: was anything inflaming my body when this blood was taken?
Iron deficiency or inflammation? How the markers move
Here is where the full panel does work a single test cannot. Iron deficiency and anemia of chronic disease can produce a similar complete blood count, but the iron markers separate them.

Cleveland Clinic describes both ends of this. Low ferritin with high TIBC typically means iron-deficiency anemia. Low TIBC alongside low iron points elsewhere: chronic inflammation, infection, cirrhosis, malnutrition or nephrotic syndrome. The direction TIBC travels is what tells the two stories apart, because your body only ramps up transferrin production when iron is genuinely scarce.
Reference ranges differ between laboratories, so compare your result to the range printed on your own report rather than one you found online. Cleveland Clinic gives a general normal TIBC of 240 to 450 mcg/dL and says plainly that labs often use different reference ranges.
Both patterns can also coexist. A woman with heavy periods and an inflammatory condition can be iron deficient and inflamed at once, which is exactly the case where transferrin saturation below 20 percent carries more weight than a reassuring ferritin.
Do you actually need the full panel?
Ask for ferritin alone if you are otherwise well and want to know whether low iron explains your symptoms. It falls first, before a full blood count notices anything, and it is the cheapest useful answer.
Ask for the full iron panel if any of these apply:
- Your ferritin came back normal, but fatigue, breathlessness or hair shedding have not budged.
- You have an inflammatory, autoimmune, kidney or liver condition, or a recent infection.
- Your ferritin sits awkwardly between 30 and 100 ng/mL and you have symptoms.
- Your doctor is separating iron deficiency from anemia of chronic disease.
- You have been treated for low iron and it keeps coming back.
Skip both if you have no symptoms and no reason to suspect a problem. An iron panel run out of curiosity mostly produces borderline numbers that need chasing.
How to read your results with your doctor
Bring three things to the appointment: the printed report with its reference ranges, a note of any illness or flare in the weeks before the draw, and a list of supplements you take. Iron tablets in the days before a test can lift serum iron, so tell your doctor if you have been taking any.
Then ask the questions that change what happens next:
- Was I inflamed when this was taken? If yes, the ferritin number needs reading against the higher threshold, not the standard range.
- What is my transferrin saturation? If it is below 20 percent, that supports iron deficiency even when ferritin looks fine.
- Why am I low? This is the question that matters most. The NHS names heavy periods and pregnancy as very common causes in women, alongside gut bleeding from ulcers, NSAIDs, bowel inflammation or piles. Treating the iron without finding the cause is half a job.
- When will you recheck? Stores rebuild slowly, and a repeat test is how you confirm treatment is working.
A note on ranges: a result at the bottom of a lab's normal range is not automatically fine, particularly for ferritin. The published deficiency cutoffs and a lab's reference interval are different things, and they often disagree.
What to do with the answer

The evidence supports reading the markers together rather than trusting one. The same review sets out why that number stops working when inflammation is present: ferritin is an acute-phase reactant, and a level between 30 and 100 ng/mL can still mean deficiency. The Clinical Medicine review reaches the same conclusion and uses transferrin saturation to settle the cases ferritin leaves ambiguous.
That is the argument for the full panel. Serum iron, transferrin saturation and TIBC each cover a gap ferritin cannot, and the direction they move is what separates iron deficiency from anemia of chronic disease.
The evidence is just as clear about the limits. No marker diagnoses anything by itself. Reference ranges differ between laboratories, results shift with recent meals, supplements and illness, and the numbers only mean something when they are read against your symptoms and your history.
Food first still matters
A panel tells you what is happening. It does not fix it. Once a cause is found and any deficiency is treated, everyday meals are what keep your stores from slipping back, and they bring protein, zinc and B vitamins that a tablet does not.
