The low-GI diet is not really a diet with rules so much as a way of choosing carbohydrates that keep your blood sugar steady. The idea is simple and useful, but it is often oversold, and a couple of caveats decide whether it is worth your attention.

What the glycaemic index means

The glycaemic index ranks carbohydrate foods from 0 to 100 by how fast they raise blood sugar after eating. The scale splits into three bands:

  • Low GI (55 or under): slow, gentle rise
  • Medium GI (56 to 69): moderate rise
  • High GI (70 or over): fast spike

A low-GI food is digested and absorbed slowly, so glucose trickles into the blood and insulin does not have to surge. A high-GI food does the opposite. The index only applies to foods with carbohydrate, so meat, fish, eggs, oils and most non-starchy vegetables barely register.

Low-GI and high-GI foods

The pattern is easier to remember than the numbers.

Processing and cooking shift things. Finely milled flour raises GI, and cooking pasta soft or leaving fruit to ripen nudges it up. This is one reason the index is a guide, not a precise rule.

Why glycaemic load matters more

The GI has a real weakness: it is measured using a fixed amount of carbohydrate, not a normal portion. Watermelon is the classic example. Its GI is high, but a slice contains so little carbohydrate that its effect on your blood sugar is small.

Glycaemic load fixes this by multiplying GI by the carbohydrate in an actual serving. It reflects how you really eat, so where the two disagree, load is usually the more honest number.

Who actually benefits

The evidence is strongest where blood sugar control is the goal. Low-GI eating improves glucose and blood-fat levels in people with type 1 and type 2 diabetes, which is where it earns its place. Women with PCOS and insulin resistance often see benefit too, since both involve the body handling glucose less efficiently. It can also steady the energy dips and cravings that follow a high-GI meal. A meta-analysis of randomized trials in women with PCOS found low-GI diets improved insulin resistance, fasting insulin, cholesterol, waist circumference and total testosterone, while body weight, fasting glucose and free androgen index did not shift. The authors flagged small trial sizes and inconsistency between studies (Advances in Nutrition, 2020).

For general weight loss, the picture is more even. Low-GI eating is a sensible approach, but trials do not show it clearly beating other balanced diets. What you eat overall matters more than chasing GI numbers.

Making it practical

You do not need a chart on your fridge. A few habits do most of the work:

  • Choose whole or minimally processed grains over refined ones
  • Fill half your plate with non-starchy vegetables
  • Include beans, lentils and other pulses regularly
  • Pair carbohydrates with protein, healthy fat or fiber, which slows the whole meal down
  • Favor whole fruit over juice

Combining foods this way lowers the effective GI of a meal, which is why obsessing over any single food's number is rarely worth it.

If you are managing diabetes, PCOS or insulin resistance, ask your doctor or a dietitian how a low-GI approach fits your treatment, especially if you take medication that affects blood sugar, so your food and medication work together rather than against each other.
